Come to your exam prepared!
If you are going to be a first-time patient of ours, the first thing we have to say is welcome to the family! We can’t wait to see you in our office. Before you visit us, please fill out this form and bring it with you to your appointment.
Pupil dilation is an optional part of the examination, so if you need a ride home, please prepare ahead of time.
Color blindness tests are also available upon request.
